Wheaton College Affiliate Wheaton Identification Application Form

This is an application form to obtain a Wheaton ID for someone who is affiliated with the college but is not otherwise in Banner as a student or employee. This is the first step in obtaining access to electronic services that are either available by having an ID card (Library privileges, door access) or through Information Technology and Services (email, WINDOW, Banner, OnCourse). In addition to your accepting responsibility for the affiliate, the form needs to be approved by a department chair, department head, or senior officer of the college. Upon approval of completed forms, you will be contacted when the services you requested are completed. If your affiliate requires a Wheaton ID card, it can be obtained at Public Safety the day following the creation of the "W"ID.
