As a new student, you will be required to submit a series of health forms via our health portal. All forms must be submitted by July 15 for the fall semester. If you are starting in the spring semester, forms are due by December 15. Late submissions may result in a hold on your account, which would impact your course registration and housing.
NOTE: If you are a student-athlete, you’ll be prompted to submit additional forms in the portal. Please contact Abby Egginton at 508-286-8210 with any questions or concerns.
Required Health Documents
All health forms can be accessed and submitted via the Magnus Health portal. Below is the list of required documentation.
- Copy (front and back) of your health insurance card
(if not using the Student Health Insurance Plan) - Vital Health Record
- Physical Exam
(All students must include evidence of a physical exam. Athletes must use the form provided by the college.) - Immunization Record
- Immunization Exemption Form
- Tuberculosis Risk Questionnaire
- Release of Information Forms for Student Affairs Offices
- Sturdy Memorial Hospital Permission for Release of Disposition Information
NOTE: Most of the health forms do not need to be completed by your doctor. We ask that you start working on them as soon as you have access to the Magnus Health portal.
It can often take several months to get in for a physical exam. If you have not had a physical exam in the past year, please call to schedule an appointment as soon as possible to ensure you see your doctor prior to the start of the semester.
If you are unable to schedule a physical exam before the deadline, please submit the remaining forms and include a note indicating the date of your scheduled physical. Then, forward the results of your physical exam as soon as possible.
Immunizations
The State of Massachusetts does not require the COVID-19 or influenza vaccines. However, we strongly recommend them to help protect the health and safety of the Wheaton community.
The Massachusetts Department of Public Health requires the following immunizations:
- Tdap (Tetanus/Diphtheria/Pertussis) — 1 dose within the last 10 years
- Hepatitis B3 dose series or positive titer proving immunity
- MMR (Measles, Mumps, Rubella) — 2 doses or positive titer proving immunity
- Varicella (Chickenpox) — 2 doses or positive titer proving immunity or physician confirmation of history of disease
- Meningococcal (Meningitis) — 1 dose of the Men ACWY (formerly MCV4) is required for all full-time students 21 years of age or younger. The dose must have been received on or after your 16th birthday. Doses received at a younger age do not count toward this requirement. You may decline the Men ACWY vaccine after reading and signing the MDPH Meningococcal Information and Waiver form provided in the health portal. Meningococcal B vaccine is not required.
If you have not completed a multi-dose series of the required vaccines noted above, receive as many doses as medically advised before the start of the semester. Any outstanding doses may be administered during the academic year at the Student Health Services Office or at Sturdy Health Primary Care–Norton. Deductibles and copays apply and all charges will be billed to your insurance. Failure to follow through with a plan for obtaining additional required doses will result in a hold being placed on your account.
International Students
All students are required to follow Massachusetts immunization requirements. If your home country’s immunization requirements differ, you may be required to receive additional vaccines.
Study Abroad Vaccines
If you plan to study abroad during your time at Wheaton, check CDC recommendations for international vaccine requirements based on your destination. Routine immunizations are available at Sturdy Health Primary Care–Norton by appointment. Some vaccinations required for traveling abroad may require an appointment at one of the travel clinics in the area:

Medical Exemptions
To opt out of any immunizations you must complete a Medical or Religious Exemption form through the Magnus Health portal. If there is an outbreak of any vaccine-preventable diseases for which you are not vaccinated, you will be required to leave campus until the risk of spreading the disease has passed.
