All members of the Wheaton community who have physical injury concerns can be evaluated by Wheaton's athletic training staff in the Haas Athletic Treatment Center. Following an injury assessment, the staff will offer recommendations for care. If extended rehabilitation or treatment is required the person may be referred to a physical therapy office. This service is available Monday through Friday by appointment only. People interested in making an appointment can do so by contacting Greg Steele at 508-286-3986 or by e-mail at gsteele@wheatonma.edu.

Forms are available here for printing. The forms are in Adobe Acrobat (pdf) format -- you will need Acrobat Reader to view and print them. (If you do not have Acrobat Reader, you can download it free from Adobe.)
The insurance form must be completed yearly and faxed or mailed to Greg Steele. Completed forms should be faxed to 508-286-5657 or mailed to Greg Steele, Head Athletic Trainer, Wheaton College, 26 East Main Street, Norton, MA 02766.
